The 18-year-old touched down in a commercial airliner at Dallas/Fort Worth International Airport on Thursday.
The unshaven fugitive was led by uniformed officers through the building with his hands behind his back.
Mexican officials put him on the flight in Mexico City after he had been in immigration custody for 28 days.
Couch formally agreed to drop an appeal against his deportation on Monday.
Mexican immigration officials said Couch "will be met by US authorities and face various charges" when he arrives in Dallas.
He and his mother, Tonya Couch, were detained in the resort of Puerto Vallarta in December after a two-week manhunt.
His mother, who is facing a felony charge of hindering apprehension, was sent back to the US and was released after posting bail.
She could face up to 10 years in prison if convicted of helping her son evade the law.
Authorities have said they suspect the pair fled to Mexico in November as prosecutors investigated a video uploaded on social media which appeared to show Ethan Couch among a group of young men playing a drinking game.
Under the terms of his probation he is not allowed to drink alcohol.
Ethan Couch was 16 when he crashed his pickup into a 24-year-old woman whose vehicle had broken down, killing her and three others who had stopped to help.
His blood alcohol level was nearly three times the legal limit.
The teenager was spared a custodial sentence after his legal team argued his wealthy upbringing had made him irresponsible, a condition termed "affluenza".
